ROIs: Patients of Ei are required, upon registration, to provide written consent for approving or denying care coordination with Primary Care Physicians (excluding instances of crisis or emergency). Ei also requires active clients to sign an ROI when and if it is necessary to coordinate or consult with other providers and professionals. This means that with active clients, we may not need to receive an ROI from you, as we may already have one on our end, but for situations where this is not the case (i.e., a client no longer in our care or who is not established), or perhaps other concerns surrounding an active client, please fax your ROI. If you do not have the ability to fax, then please call our provider line or submit a Provider Contact form, requesting a Bitwarden password link. You will need to provide an email or phone number where this link can be sent. After you receive this password protected link, you will be able to upload the ROI into our electronic Secure Vault.
Receiving Documents: Documents containing sensitive information can be uploaded to our electronic Secure Vault. A Bitwarden link will be emailed or texted to you so that you can upload documents to this vault. The Bitwarden link will be sent to the email or phone number listed in the signed ROI.
Sending Documents: To keep as little PHI as possible from being handled outside of our ERM, we construct ROIs, Care Coordination, Referral Requests, and Summary of Care documentation within our ERM software, and then send these documents from our ERM through built-in fax features.
